All applications require copies of the following:

• Drivers License
• Fundamentals of Coaching - Course Certificate
• Sudden Cardiac Arrest - Course Certificate
• Concussion in Sports - Course Certificate
• Heat Illness Prevention - Course Certificate
• Mandated Reporter - Course Certificate
• Sexual Harassment - Course Certificate
• Valid First Aid and CPR Certificate for Infant/Child/Adult, issued by a recognized agency
• TB Test

Please submit applications by the deadline listed on the posting above OR at least one month prior to the start of the sports season, whichever occurs first. Applicants will not be eligible to coach until all forms have been completed and received.
